IPD: Clinical Charting

1:1 Observation Scheduler

The 1:1 Observation Scheduler (ipd/pages/obs_scheduler.php) manages close-observation staffing for high-risk patients (e.g., active suicidal ideation, high fall risk). It auto-suggests staffing ratios based on real-time risk data, and lets charge nurses assign, track, and reassign observation duty across shifts. Requires ipd_nursing_charts.

Shift definitions used here

Morning: 6am–2pm · Afternoon: 2–8pm · Evening: 8–10pm · Night: 10pm–6am (note: slightly different boundaries from the general nursing shift used elsewhere in IPD).

Step-by-step

  1. Go to 1:1 Observation Scheduler.
  2. Review the auto-suggested staffing ratio for each at-risk patient, based on their current risk level.
  3. Assign a specific staff member to each observation slot for the current/upcoming shift.
  4. Reassign as needed if staff availability changes mid-shift.
  5. Track completion/coverage across the day.

Common issues & fixes

the suggestion is risk-based and pulls from the patient's latest risk assessment; if that assessment is outdated, update it first (see Risk Assessments).
check they're marked available/on-shift; the scheduler pulls from active ward staffing.
