Assessments & Rating Scales

The Assessments page (?page=assessments) lets clinicians administer standardized psychiatric rating scales (e.g., depression/anxiety inventories) as well as clinic-defined custom scales, and review a patient's assessment history with automatically derived severity interpretation.

Step-by-step: administering a scale

  1. Go to Assessments.
  2. Select the patient.
  3. Select a scale from the built-in list or the "Custom Scales" group.
  4. Answer the scale's questions as they appear.
  5. Submit — the system scores it and shows the severity band based on that scale's defined thresholds.

Viewing history

  1. Switch to the history view.
  2. Filter by patient and/or scale.
  3. Review past results and severity trends over time.

Requesting a new custom scale

There's a request form to describe a scale you need built — an admin (or the system's AI-assisted scale builder) can add it as a custom scale available clinic-wide.

Common issues & fixes

it may need to be added as a custom scale by an admin.
severity is derived from that specific scale's own interpretation thresholds; different scales use different cut-offs.
