Ligature Risk Register

The Ligature Risk Register (ipd/pages/ligature_risk.php) is a ward/room-level hazard checklist for identifying and tracking ligature risks (fixtures or features that could be used for self-harm) — mandatory under NABH accreditation standards for psychiatric units. Admin-only (requires ipd_settings).

Step-by-step

  1. Go to Ligature Risk Register.
  2. Add a new risk entry: select the ward, describe the specific location, the risk item identified, its category, and risk level.
  3. Record who identified it and when.
  4. Document a mitigation plan and its status (e.g., pending, in progress, resolved).
  5. Set a next review date to ensure the item isn't forgotten.
  6. Save — the register builds a full audit trail of hazard identification and remediation over time.

Common issues & fixes

always physically verify mitigation before marking an item resolved, since this register may be reviewed during accreditation audits.
periodically filter/sort by next review date to catch items that need re-assessment.
