Craving & Relapse Tracker

The Craving & Relapse Tracker (ipd/pages/craving_tracker.php) records daily craving intensity using a Visual Analog Scale (VAS), maps triggers, tracks coping strategies used, and logs any relapse events during the admission. Requires ipd_nursing_charts.

Step-by-step

  1. Go to Craving & Relapse Tracker for the admission.
  2. Log a new entry: substance, craving score (VAS), the trigger and its type, and any coping strategy used.
  3. If a relapse occurred, mark it and record the substance and amount involved.
  4. Note whether the entry was self-reported by the patient or observed/recorded by staff.
  5. Review the trend over time to spot patterns in triggers or high-risk periods.

requires ipd_nursing_charts.
a logged relapse should prompt a review of the treatment plan and possibly a risk reassessment; don't let it sit as a data point only.
try to use the same trigger-type options consistently across entries so trend analysis stays meaningful.
